Medical Training Initiative (MTI)
Update on the Medical Training Initiative: 1 April 2026
NHS England have confirmed that the current national contract with the Academy of Medical Royal Colleges (AoMRC), through which MTI is delivered, will end on 31 March 2026 and will not be renewed.
This decision follows a review of the MTI programme and an options analysis which concluded that the scheme, in its current form, is no longer consistently achieving its original objectives. As a result, NHS England has taken the decision to pause funding for all new MTI cohorts from April 2026.
Unfortunately, this means that RCPsych will no longer be able to facilitate any new MTI placements. Sadly, this means that we will not be able to progress the applications of doctors who were previously found appointable to the scheme. We recognise the commitment, preparation, and time these doctors have invested, and sincerely apologise for the distress and disruption this decision may cause.
This decision does not affect doctors currently in MTI placements in the UK, NHS England has made clear that these doctors will be fully supported to complete their placements as planned. RCPsych will remain closely involved with MTI doctors and their employers and will continue to provide them with guidance and support.

The Medical Training Initiative has, for many years, enabled doctors from partner countries to develop skills in the NHS before returning home to contribute to their local health systems, and we are proud of the role psychiatry has played in that endeavour. Please be assured that the College remains committed to supporting international psychiatrists wherever possible, and we will continue to explore alternative routes for clinical training and professional development.
